WOONSOCKET, R.I. — With the U.S. population of adults 65 and older growing at a pace far outstripping the overall population, CVS Health is highlighting new models of care for older adults through its Oak Street Health, Signify Health and MinuteClinic operations.
According to recent census data, the 65-plus population has been expanding five times faster than the population overall, raising demand for services that address chronic conditions such as heart disease, diabetes, arthritis and dementia. CVS Health’s business units are emphasizing approaches designed to make care more accessible and better coordinated.
Oak Street Health: Community-based primary care
Oak Street Health operates primary care centers in underserved areas, combining physical, behavioral and social health services. Patients are primarily Medicare beneficiaries managing multiple conditions. Company data show the model has led to a 44% reduction in hospital admissions compared with similar Medicare populations.
Signify Health: In-home evaluations
Signify Health provides In-Home Health Evaluations, often covered by Medicare Advantage plans, in which clinicians assess chronic conditions, review medications, and screen for factors such as fall risk, cognitive decline or social isolation. Findings are shared with providers and health plans to improve follow-up care.
MinuteClinic: Age-friendly practices
MinuteClinic, the largest U.S. retail clinic network, has adopted the “4Ms” framework of age-friendly care — What Matters, Medications, Mentation and Mobility. Since 2021, more than 342,000 older adults have received medication screenings, over 6,000 patients with mobility concerns have been identified, and more than 422,000 depression screenings have been conducted.
These initiatives highlight how large healthcare providers are adapting to the demographic shift toward an older patient base, with a focus on both managing chronic illnesses and addressing broader quality-of-life issues.
