WOONSOCKET, R.I. — CVS Health has named Creagh Milford, DO, as president of Oak Street Health. He replaces Brian Clem.
"Brian Clem has decided to move on from Oak Street Health at the end of May so he can spend more time with his family. We’re grateful for his years of tireless leadership and dedication. Creagh Milford, has been named president of Oak Street Health, effective immediately. Dr. Milford will continue to serve as interim president of Retail Health while we conduct a search for a new leader," CVS said in a statement.

Dr. Milford has been with CVS since 2021, most recently serving as president of retail health.
“I’m excited to start a new chapter at CVS Health as President of Oak Street Health, where I have the honor of developing and executing business strategy to further grow the organization,” he wrote on his Linkedin page. “As a practicing physician, I am passionate about providing consumer-centric primary care to patients — and Oak Street Health shares that passion, resulting in better experiences and outcomes for the older adults we serve.”
"Although I’m serving as interim President of Retail Health for the near future, I want to publicly express my deep appreciation to my Retail Health and MinuteClinic colleagues I’ve worked alongside for the last 3+ years – where we served more than five million patients last year. I’m proud of the work we’ve done to evolve our business strategy and offer primary care at select MinuteClinic locations across the U.S. With Retail Health and Oak Street Health both being part of CVS Heath’s Health Care Delivery business led by Dr. Sreekanth Chaguturu, I’ll be able to witness the team’s continued dedication and commitment to improving health care at the community level," he wrote.
"Lastly, I want to extend my personal gratitude to Brian Clem and the entire Oak Street Health team for its dedication to serving patients and helping create a premier value-based care model in the country. It’s truly impressive what you’ve accomplished and in such a short time. I look forward to working with – and learning from – our Oakies and together collaborating with colleagues across the enterprise to harness the power of our combined CVS Health assets and drive meaningful results for years to come. Later this week I’ll have the opportunity to meet and engage with more colleagues at our Oak Street Health Town, during which Brian and I will have a conversation about the tremendous possibilities ahead," he concluded.
