Oak Street Health emphasizes preventive care this fall

Oak Street Health urges older adults to review preventive care this fall, including wellness visits, vaccinations and recommended screenings.

CHICAGO — Oak Street Health is encouraging older adults to use the fall months to review their preventive care needs, including annual wellness visits, vaccinations and recommended health screenings.

Jean Reidy, an adult nurse practitioner at Oak Street Health, said the end of the year provides an opportunity for older adults to assess their health and make sure they are up to date on preventive care.

“I encourage older adults to think of this time of year as a health check-in. Make sure you’re up to date on your Annual Wellness Visit, recommended vaccines and preventive screenings,” Reidy said. “These appointments can help identify health concerns early and provide a clear plan for staying healthy in the months ahead.”

During a Medicare Annual Wellness Visit, providers review a patient’s medical history, update the individual’s care plan, and check health indicators such as blood pressure. The visit may also include screenings for depression or cognitive changes.

Reidy also recommends that older adults speak with their healthcare providers about vaccines that may be appropriate for them, including the flu, COVID-19, shingles, pneumonia and RSV vaccines. Recommendations can vary based on age, medical history and individual health needs.

Preventive screenings should also be discussed with a healthcare provider. Depending on a patient’s age and risk factors, screenings may include those for colon, breast, prostate, and lung cancers.

Reidy said mental health should remain part of preventive care, especially during the colder months, when shorter days and reduced social interaction can contribute to isolation or depression. Oak Street Health screens patients for depression and provides access to behavioral health specialists who work alongside primary care providers.

“Don’t wait until something feels wrong to prioritize your health,” Reidy said. “Preventive care can help you stay ahead of potential health concerns and give you the resources and support needed to maintain your health and independence. A few simple steps today can make a meaningful difference in the months and years ahead.”
